DUKA 2025 Registration Form

Notes:

1. The Registration Fee is necessary to secure your Costume and/or T-Shirt to participate in the Band.

2. The Registration Fee is an advance which is deductible from the Costume and/or T-Shirt cost, and non-refundable unless DUKA Mas Domnik UK Carnival Band decides not to participate in Carnival 2025.

3. Each costumed Masquerader and T-Shirt Section member must register individually for Health & Safety Risk Assessment and Insurance purposes.

4. ⁠Masqueraders undertake to pay the cost of the costume selected, or total fee for participating in their Own Costumes.
